WASHINGTON —The U. S. Food and Drug Administration approved a generic version of Celebrex, a painkiller used to treat ulcers, for sale next month, and the drug has already shipped to more than 1.5 million patients nationwide.
The drug is currently being developed by AstraZeneca, a biopharmaceutical company based in Cambridge, Mass., and is the first non-surgical treatment for a painful condition known as ulcers.
“Today’s FDA decision not only makes the drug more widely available and cheaper but it also helps to ensure that patients with chronic ulcers don’t experience severe symptoms,” said FDA commissioner John Votek, in a statement.
Celebrex, an FDA-approved drug, has been available since 2002 and has been prescribed by patients since 1987.
It’s been used to treat ulcers in more than 100 million people worldwide. The drug’s label also lists ulcers in the form of an antibiotic called ciclosporin, which is a type of immunosuppressant. The ulcer risk for Celebrex has been linked to an increase in ulcers.
The drug was approved on Nov. 13, 1998, but was later withdrawn after a study found that the drug had a 30% to 50% lower risk of developing a serious ulcer.
Celebrex was sold by AstraZeneca in the United States in 2003 and in Europe in 2005. It’s now available in more than 100 countries, including the United States, as well as the United Kingdom, Spain, Germany, and Israel.
The FDA approved Celebrex in 2003 and the drug’s label has been updated since that time to reflect a new drug class called COX-2 inhibitors. The FDA also removed the drug’s label warning about an increased risk of ulcer recurrence.
The FDA approved Celebrex in 2004, but the warning about an increased risk of ulcers was updated to include information about the drug’s side effects. The drug had a risk of causing ulcers of the small intestine and gastric bleeding, which can occur from ulcers.
An ulcer in a young person can cause life-threatening ulcerations, ulcers in adults, or ulcer bleeding that does not stop after the age of 65. The risk of ulcers may increase if the person has a stomach ulcer, ulcers in the stomach, or bleeding from the anus.
In the first two weeks after the FDA approved Celebrex in 2004, the average age of the patients who developed ulcers was 60, compared with 82 in the year before Celebrex was approved. In the first four weeks after Celebrex, ulcers were more common in patients with a history of stomach ulcers.
About three out of every 100 patients prescribed Celebrex in the United States are taking it. But many of these patients don’t realize that the drug’s risks are greater than the risk.
“I’m confident that the decision to approve Celebrex for this drug will be the same as any other that has ever been approved,” Votek said.
The FDA approved Celebrex in 2003, but it had to withdraw it in June 2004, after a study showed that the drug had a lower risk of causing a dangerous complication of ulcers than other NSAIDs.
That risk could be increased if patients already have ulcers, Votek said, because ulcers can be caused by the medications, not an NSAID.
Votek and his team decided to take action on Nov. 27, 2005, when the FDA ordered the drug’s labeling. The agency also sent a notice of voluntary recall to consumers, in a move that made Celebrex available without a prescription in the U. S., the drug’s manufacturer.
Celebrex is now available to patients without a prescription and is expected to be sold by the end of 2006.
The FDA approved the drug’s label in July 2005, and it’s now available to patients and doctors as well as pharmaceutical companies, including Pfizer, in the U. and Europe.
Celebrex was prescribed to many patients in the U. S., and it was marketed as a painkiller, Votek said.
Pfizer also recalled Celebrex from more than 2 million patients worldwide, as well as the manufacturer from Canada, in November 2004. The recalled product is being recalled, Votek said.
